Immunizations

The law requires that students receive a minimum number of immunizations prior to entering school. The law is meant to prevent illnesses such as mumps, pertussis (whooping cough), and other vaccine-preventable diseases from spreading and harming our children. To obtain proper immunizations, call your doctor, health center, or the Milwaukee Health Department at (414) 286-3521. When your child is immunized, provide the records to your child’s school

Age/Grade Required Immunization*
Pre-Kindergarten (2–4 years)
  • 4 doses of Tetanus, Diphtheria & Pertussis
  • 3 doses of Polio
  • 3 doses of Hepatitis B
  • 1 dose of Measles, Mumps and Rubella
  • 1 dose of Varicella
K5 – Grade 12
  • 4 doses of Tetanus, Diphtheria & Pertussis
  • 1 additional dose of Tetanus, Diphtheria, Pertussis to enter grade 7
  • 4 doses of Polio
  • 3 doses of Hepatitis B
  • 2 doses of Measles, Mumps and Rubella
  • 2 doses of Varicella
  • 1 dose MenACWY-containing vaccine for entry to grade 7
  • 1 additional dose of MenACWY-containing vaccine for entry to grade 12
