Immunizations

The law requires that Milwaukee Public Schools (MPS) students receive a minimum number of immunizations prior to entering school.

The law is meant to prevent illnesses such as mumps, pertussis (whooping cough), and other vaccine-preventable diseases from spreading and harming our children. To obtain proper immunizations, call your doctor, health center, or the Milwaukee Health Department at (414) 286-8034. When your child is immunized, provide the records to your child’s school.

To get free immunizations or check your family’s immunization records, contact your health care provider or the City of Milwaukee Health Department at (414) 286-8034. You can also look up records using the Wisconsin Immunization Registry.

Required Immunizations

Immunization K3 and K4 Grades K5–12
Tetanus (lockjaw), Diphtheria,
and Pertussis (whooping cough)
(DTaP/DTP/DT/TD)		 4 doses

4 doses, plus 1 additional dose
for entry to Grade 7
Polio 3 doses 4 doses
Hepatitis B 3 doses 3 doses
Measles, Mumps, and Rubella
(MMR)		 1 dose 2 doses
Varicella (chickenpox) 1 dose* 2 doses*
MenACWY-containing vaccine 
(meningitis) 		  

1 dose for entry to Grade 7

1 additional dose for entry to Grade 12** 

*Exceptions to the varicella vaccine requirement will be allowed in both childcare centers and schools only if the child has had a case diagnosed by a qualified health care provider. 

**A second dose is not required for students who received their first MenACWY-containing vaccine dose at age 16 or older.

Note: State law requires all public and private school students to present written evidence of immunization against certain diseases. Age- and grade-specific requirements can only be waived if a properly signed health, religious, or personal conviction waiver is filed with the school within 30 school days of admission.
